Community Development Long Island (CDLI) is a proud partner of the Health Empowerment Alliance of Long Island (HEALI), Long Island’s Social Care Network (SCN) under the New York State Medicaid 1115 Waiver. HEALI is a regional network of more than 100 community-based organizations, healthcare providers, and social service partners working together to connect Medicaid members with services that address health-related social needs. Through this collaborative network, individuals and families across Nassau and Suffolk Counties can access coordinated support that improves health, safety, and overall quality of life. As part of the HEALI Social Care Network, CDLI provides services that help identify needs and connect Medicaid members to critical supports, including: Health-Related Social Needs Screening A short screening to identify health and housing needs that may qualify for support. Navigation and Care Coordination Help connecting you to the right local services and providers. Enhanced Case Management Ongoing support to help you access and stay connected to needed services. Asthma Remediation Home improvements that reduce asthma triggers such as mold, pests, and poor ventilation. Asthma Self-Management Education Education and guidance to help you better manage asthma at home. Home Remediation Services Repairs and improvements that make your home healthier and safer. Home Accessibility Modifications Modifications such as ramps, handrails, or grab bars to improve safety and mobility. These services are designed to address factors that impact health outside of the doctor’s office—such as housing conditions, environmental triggers, and accessibility challenges—so individuals can live more safely and independently.
